Q:How should I file a claim?

A:Claim forms began mailing on December 1, 2023, and will continue to be sent out on a rolling basis. You can scan the QR code provided on the claim form and use the credentials provided on the form to submit your claim. Alternatively, you can mail the claim form to the specified address. If you choose to submit your claim online, you can visit the official claims website (www.paymentcardsettlement.com) and enter the Claimant ID and Control Number provided on the received claim form, or use your taxpayer identification number and some additional information to submit. Online applications must be submitted by 11:59 PM (Pacific Time) on August 30, 2024.

If you are a MenuSifu USezpay merchant, we will provide free claim application services and submit the claim on your behalf. We will contact you via email, and you may be required to provide relevant documents needed during the application review phase.

Q:Am I eligible to file a claim?

A:Anyone business or entity that accepted Visa and/or MasterCard payments services between January 1, 2004, and January 25, 2019, is eligible to receive a portion of the settlement compensation.
